Many agencies are challenged with outdated and non-functional electronic health record (EHR) systems. Moving from one EHR to another can be clinically, operationally and financially hazardous. Discuss best practices for transitioning to a new EHR. Topics will include old and new vendor management, contracting and implementation best practices.

Learning Objectives:

• List the primary reasons for changing EHR systems and describe the major challenges faced by agencies when transitioning to a new EHR and plan for facing each of these challenges
• Conduct an EHR Needs Assessment using a comprehensive functional needs assessment tool
• Create an action plan for transitioning to a new EHR that addresses the major challenges
